Case Management provides assessment, planning, support and assistance for accessing supports required to maintain the care relationship at home. The Making a Difference Program (MADP) provides flexible support packages to assist families caring for a family member aged over 6 years old with a disability at home.
The aim is to strengthen the capacities of the families to care for their family member at home.
The MADP assists families and carers by providing:
- Assistance to families who self-manage but require short term support for up to 3 months.
- Long term assistance which provides case management and support packages when required.
